I hereby apply to renew the waste disposal permit for the facility in accordance with the Environment Management Act 2005 and the Waste Disposal and Recycling Regulations 2007;
I agree to abide by all conditions imposed by the WPC Administrator;
I understand that failure to comply with permit conditions may result in a Fixed Penalty of $1,000 and other penalties under EMA 2005.
